Person Centred Planning Project
The Person Centred Planning Project works with people who are going through changes in their lives and who are referred by the Commissioning Team or Community Learning Disability Team. It helps people by working with them and those who are important to them to plan what they want to do. The project can help with individualised budgets and support brokerage.
